To apply to the MAT program, you need to submit an application to the Graduate College. A summary of the application instructions is available on line, including a link to the on-line application form.
As the instructions describe, in addition to the application you will need to submit to the Graduate College:

GRE scores: The University requires the GRE for admission to all graduate programs. The scores are good for five years, so if you took the test within the last five years, you do not need to take the test again. If you wish to submit the application before taking the GRE, specify on the application the date by which you will have taken the GRE. You must have taken the GRE test before the end of your first semester after being accepted into the graduate program.
The MAT program starts in the summer, so it's best to apply in the spring (March is a good target) to formally start the MAT program for that summer. You may begin taking the coursework for the MAT program before applying and being accepted to the program (click here to find out how). However, only nine credit hours of the MAT coursework may be transferred into the program after acceptance.
